Hermes 73fde1e2a8 [grade=B] fix(DC-162 rev3): presence-based id detection, null-params rejection, boundary matrix
Judge r2 (grade C) — all 3 blockers + both polish items:

1. REQUEST-vs-NOTIFICATION by PRESENCE, never truthiness: validateJsonRpc
   uses hasOwnProperty('id'); id 0 and id '' are VALID request ids echoed
   back BY VALUE (strict-equality tested), id:null treated as a request
   under the documented error-id convention. mcp-core default branch fixed
   the same way (`if (id)` → hasOwnProperty — id 0 / '' unknown-method now
   answer -32601 with the id echoed instead of being swallowed as falsy).
   Routes' malformed-notification silence path also switched to
   presence-based detection (id:null + bad params → 400 envelope, not 204).

2. params:null REJECTED (-32600): contract is object-or-omitted ONLY
   (strict JSON-RPC; the old code's m.params !== null guard contradicted
   the documented contract). Boundary tests pin the policy.

3. Unknown-method boundary tests: id 0 → -32601 id:0, id '' → -32601 id:'',
   id null → -32601 id:null — asserted at BOTH layer levels (route).

Polish:
- 16-case TABLE-DRIVEN boundary matrix covering every allowed/rejected
  id/params combination incl. 202 vs 204 notification split.
- Verifier now probes writable /tmp up front (exit 73 + clear message if
  unavailable; documented requirement) instead of dying at mktemp.
- Verifier target count 57 → 73 (DC-162 suites) with the boundary table
  named in the assertion label.

Full DC-162 suite: 73 passed (route 50 + core 23). No transport/credential
semantics changed from rev2.

/**
* DC-162 rev2: MCP Streamable HTTP transport — "Connect your AI" backend.
*
* Implements the MCP **2025-03-10 Streamable HTTP** transport (the spec
* revision that replaced the deprecated HTTP+SSE one) so external AI
* assistants (Claude Desktop/Code, ChatGPT connectors, Hermes, mcp-remote,
* anything spec-compliant) connect to a URL with a `dk_` API key — no local
* bridge process required; the stdio variant (src/mcp/mcp-server.js)
* remains for clients that want it.
*
* Spec conformance map (modelcontextprotocol.io/specification/2025-03-10/basic/transports):
* POST — the single client→server channel. The server MAY answer
* application/json for simple request/response (we do — this
* server generates no server→client messages, so no SSE).
* - valid request → 200 + JSON-RPC response object
* - valid notification → 202 Accepted, empty body
* - malformed request → 400 + JSON-RPC error (id null when
* unpairable)
* - malformed notification → no response body (204; JSON-RPC:
* servers MUST NOT reply to broken notifications)
* GET — server→client stream channel. We answer **405 + Allow: POST**
* (spec: "MUST either return Content-Type: text/event-stream ...
* or else return HTTP 405").
* DELETE — session termination. We are **stateless** (no session store,
* no Mcp-Session-Id issued) → 405 + Allow (spec MAY for servers
* without session management).
* Origin — spec MUST validate when present (DNS rebinding). Machine
* clients (CLI/desktop) send none → allowed; browser origin must
* match this request's Host (same-origin).
* Accept — clients MUST list application/json + text/event-stream; we
* require json to be acceptable, else 406.
* MCP-Protocol-Version header — when present must be supported, else 400.
* Batching — REMOVED in 2025-03-10. Arrays (including []) → Invalid
* Request. (r1's custom batch handling was rejected; spec wins.)
*
* Auth model (deliberate, unchanged placement):
* - The route lives INSIDE the authenticated /api/v1 namespace. No new
* PUBLIC_ROUTES entry — a stranger cannot reach it unauthenticated.
* - CSRF middleware already skips x-api-key / Bearer requests
* (src/security/csrf-protection.js ~line 216) — machine calls pass.
* - Credential TYPE preservation (judge r1 blocker): the downstream
* loopback call carries the SAME credential that authenticated this
* request (req.auth.type is set to 'apikey' | 'jwt' | 'session' by
* src/utilities/middleware.js:549-586 — verified, not guessed):
* apikey → x-api-key with the caller's dk_ key
* jwt → Authorization: Bearer <caller's JWT>
* session/none → NO machine credential: tools/call and resources/list
* deny with JSON-RPC -32001 (a dashboard cookie is not a delegable
* machine credential; mint a real key in the modal).
* Downstream per-route scope checks apply identically for both kinds;
* a read-only key cannot deploy.
* - JSON-RPC 2.0 structural validation (validateJsonRpc): jsonrpc must
* be '2.0', method a non-empty string, params object-or-omitted, id
* string|number|null when present → else -32600 Invalid Request.
* tools/call without params.name string → -32602 Invalid params.

*
* ID DETECTION POLICY (judge r2): presence-based, never truthiness —
* - id ABSENT → notification (no response body expected)
* - id present with value → request; 0 and '' are VALID ids (returned
* back on the response with strict equality)
* - id present as null → request per the JSON-RPC error-id
* convention; we answer with a response whose
* id is null. (Documented choice; not a
* notification, not a 400 — null is a legal
* member of the id union.)
* PARAMS CONTRACT: object-or-omitted ONLY. params:null is REJECTED
* (-32600) — strict JSON-RPC (spec: "params: This member MAY be omitted").
